You may complete the medical form upon arrival, but we strongly recommend reviewing it in advance
- If you answer “NO” to all 10 questions, no doctor’s evaluation is needed.
- If you answer “YES” to questions 3, 5, or 10 — or to any follow-up questions on page 2 — you must bring a doctor’s signed approval using all 3 pages of the form.
- Unfortunately, you cannot begin in-water training without the required medical clearance.

Cancellation Policy
All cancellations or reschedule requests must be received and acknowledged at least 48 hours prior to the scheduled start time. For boat trips, cancellations made 48+ hours in advance are eligible for a full refund (including deposit). Cancellations made within 48 hours are non-refundable. No refunds will be issued for unused services or rental equipment. Tanks and weights have no cash value, and bringing your own does not reduce the trip price.
For courses, at least 48 hours’ notice is required to reschedule or cancel. Deposits are non-refundable. Required eLearning must be completed prior to arrival—failure to do so will require rescheduling and incur a $119 rescheduling fee.
All trips and training are weather and condition dependent. The Captain and management reserve the right to cancel trips or change dive locations if conditions are deemed unsafe. If a dive trip is canceled due to weather, a full refund will be provided. For courses, rescheduling or alternative training options will be offered.
