Thank you for booking with **Aloha Scuba Diving Company**. We look forward to your dive. Feel free to contact us with questions.
Please arrive 30 minutes early before your departure time for a mandatory orientation. For the morning departure please arrive at 8:30AM and 1:30PM for the afternoon departure.
**We leave at 9AM *SHARP* for the morning tour and 2PM *SHARP* for the afternoon tour**
Important Information
A completed [PADI Diver Medical Participant Questionnaire](https://cdn.filestackcontent.com/Uu4ZUmDwTlykpD4e0jIW) is also required for participation. If you answer ‘YES’ to questions 3, 5, or 10 on the form, or to any of the questions on page 2, you must sign and date the participant statement and take all three pages of the form to your physician for a medical evaluation.
